If you are an established patient at Tower Clock Eye Center, you may access your records through our web portal.  To view your records, go to www.myeyecarerecords.com and register.  You will need to use your name exactly as it is entered in our medical records.  If you need assistance, please contact us.

Referring Doctors

Please fax completed form to us prior to your patient’s scheduled appointment, or you can have your patient bring it on the date of his/her visit. Please make photocopies for your future reference.
